Trans Abstract

Objectives:

As the surgical techniques are developed such as adopting robotic surgery etc. It has been reduced postoperative side effects such as urinary incontinence and sexual dysfunction etc., but also remain constant risk of recurrence after the treatment. It is very important to figure out the predictive factors, whichincreasing the probability of long-term survival, by Analyzing the progression of the disease according to biochemical recurrence (BCR). This study aims to figure out the influential predictive factors on BCR by using Korean Prostate Cancer Registry (KPCR) database, which constructedwith providing data from Korean manifold, because of therecurrence-free survival rates are reported to vary according to geographical location and race.

Methods:

KPCR database consists 7,394 patients’ data which collected from six medical institutions, and final analysis was conducted with 5,119 patients’ data which collected from December 2003 to December 2014. Statistical analysis was conducted with the multivariable Cox proportional hazard regression models to figure out the correlation of clinical factors which affect BCR. The biochemical recurrence free survival rate (BCR-FS) was calculated using with Kaplan-Meier method.

Results:

The 5 year BCR occurred in 23.6% (1,209 patients). As a result of the recurrence rate, prostate specific antigen (PSA) value ≥20.1 ng/mL was 49.3%, the clinical T3 stage was 40.4%, Gleason score ≥8 was 54.4%, Gleason score 7 was 21.8%. The mean duration of BCR was 20.7 months and the strongest predictor of BCR was Gleason scoreand PSA value ≥20.1 ng/mL.

Conclusions:

It has been verified that, Gleason scores and PSA value are the most influential factors for biochemical recurrence after radical prostatectomy. These results will be helpful fordetermining the treatment direction in postoperative follow-up and able to contribute to improving survival outcome in the future.

서 론

전 세계 남성에서 두 번째로 흔히 진단되는 전립샘암은 미국남성에서 1위, 한국남성에서 5위로 발생한다[1,2]. 초기에 발견되는 국소 전립샘암은 기대여명이 10년 이상이고 완치 가능한 환자에서 표준치료로 수술적 전립샘절제술(radical prostatectomy)을 시행하고 있다[3,4]. 국소 전립샘암의 치료에 로봇수술의 도입 등 수술 술기가 발달함에 따라 요실금, 성기능장애 등 수술 후 부작용은 감소하였지만 치료 후 재발의 위험은 상존한다[5]. 전립샘 암수술 치료 후 추적검사에서 전립샘 특이항원(prostate-specific antigen, PSA)값이 측정 불가능한 수준으로 측정될 경우 잔여암이 없는 상태를 나타내는 것으로 생각되어 왔고 PSA값이 증가할 경우 잔여암 또는 재발암을 가장 먼저 나타내는 증거로 사용되어 왔다[6]. 이 같은 PSA 상승은 생화학적 재발(biochemical recurrence, BCR)로 정의되며 5년 재발률이 23%, 10년 재발률이 35%, 15년 재발률이 44%로 보고되고 있다[7-9].

환자의 치료방법을 선택할 때에는 환자의 연령, 기대여명, 암의 악성도, 전신적인 건강상태 그리고 삶의 질에 관련된 환자의 선호도 등과 함께 예후에 대한 판단을 종합하여 최종적인 치료방침을 결정하게 된다. 이때 예후에 대한 결과를 예측할 수 있다면 치료방침을 보다 합리적으로 결정할 수 있을 것이다. 이러한 판단을 위해 국소 전립샘암의 치료 후 BCR에 따른 질병의 진행양상을 분석하여 재발에 영향을 미치는 예측인자를 파악하고 확인된 예측인자를 통해 BCR을 조기에 발견하고 치료에 도움을 주는 것은 장기 생존 확률을 높이는 데 매우 중요하다[10]. 이러한 분석은 대규모의 전립샘암 데이터를 이용할 때 보다 객관적인 결과를 제공할 수 있으며 재발예측의 정확도를 높이고 환자와 의료진에게 전립샘암의 특징 이해와 치료방침의 결정에도 도움을 줄 수 있다.

이 같은 BCR의 예측인자를 분석하는 연구는 지속적으로 이루어져 왔으나 지리적 위치와 인종에 따라 생화학적 무재발 생존율(biochemical recurrence free survival, BCR-FS)의 차이가 보고되고 있기 때문에[11,12] 한국인 환자에게 적용했을 때 잘 예측되지 않을 수 있다. 그러므로 한국의 다기관 코호트 연구를 통해 한국인에게 최적화된 예측 결과를 제공하는 것이 중요하다. 다양한 임상 및 병리인자들이 BCR을 예측하는 변수로 확인되었고 국내외 연구에서 밝혀진 변수로는 술전 PSA, 병리학적 T 병기, Gleason Score, 림프혈관침범(lymphovascular invasion, LVI), PSA 밀도(prostate-specific antigen density, PSAD), 절제변연(surgical margin, SM) 등이 제시되었다[13-17].

이에 본 연구에서는 다기관에서 제공받은 자료로 구축된 한국의 전립샘암 데이터(Korean Prostate Cancer Registry, KPCR) 데이터베이스를 활용하여 BCR이 발생한 환자의 임상 및 병리학적 결과를 분석하고 한국인에서 BCR에 영향을 미치는 예측인자를 알아보고자 한다.

연구 방법

연구대상자의 특성

이 연구에서 활용한 데이터는 KPCR 데이터베이스로 한국의 6개 의료기관에서 2001년 5월부터 2014년 12월까지 근치적 전립샘절제술을 받은 7,394명의 전립샘암 환자 데이터가 적재되어 있다. 이 데이터베이스에 조건을 적용하여 분석 자료를 추출하였고 2003년 12월부터 2014년 12월까지 5,119명의 전립샘암 환자 데이터로 구성하였다. 추출조건은 초기치료로 근치적 전립샘절제술을 시행받고 최종 추적기간이 12개월 이상인 한국남성을 대상으로 하였다. 제외기준으로는 외국인 51명, 최종 추적기간이 12개월 미만 633명, 초기치료가 수술이 아닌 자 80명, 보조요법을 시행자 230명, 분석에 사용하는 항목이 한 가지라도 없는 환자 1,281명(수술 전 PSA [7], 임상학적 T 병기 [8], Gleason Score [62], 병리학적T 병기[1], SM [435], 신경침범[perineural invasion, PNI] [459], 정낭침[seminal vesicle invasion, SVI] [216], 피막외 침범[extracapsular extension, ECE] [17], LVI [76])을 제외하고 최종 5,119명을 대상으로 BCR이 발생한 환자를 대상으로 임상 및 병리학적 결과와 예후에 대한 분석을 실시하였다(Figure 1).

분석방법

임상 및 병리학적 일반적인 특성은 chi-square test와 one-way ANOVA 검정으로 분석하였다. 임상 및 병리학적 변수는 비뇨기과전문의의 자문을 통해 구성하였으며 PSA 수치의 범주와 Gleason Score의 단계는 미국암연합회(American Joint Committee on Cancer, AJCC) 제7판의 예후분류 단계를 기반으로 하였다. BCR에 영향을 미치는 임상인자와의 상관관계는 다변량 Cox 비례위험 회귀분석(multivariable Cox proportional hazard regression models)을 통해 알아보았으며 비례위험도 가정에 대한 검토는 log minus log (LML) curve와 Time-dependent Cox regression 분석법을 통해 비례위험도의 가정 검토를 실시하였다. 그리고 Kaplan-Meier method를 사용하여 BCR-FS를 구하고 병리학적 T 병기별로 그 차이를 분석하였으며 생존율의 차이는 log-rank test를 통해 확인하였다. 모든 통계분석은 SPSS 18.0 (SPSS Inc., Chicago, IL, USA)을 사용하였고 통계적 유의성은 유의수준 0.05 미만의 경우 유의한 것으로 판단하였다.

본 연구 프로토콜은 가톨릭대학교 임상연구심사위원회의 지침에 따라 승인되고 수행되었다(IRB No. MC16RIMI0107).

연구 결과

연구대상자의 특성

전체분석 대상자인 5,119명 중 BCR은 23.6% (1,209명)에서 발생하였고 임상·병리학적 특성은 Table 1과 같다. BCR이 발생한 환자에서 수술 전 PSA값이 ≥20 ng/mL에서 재발률은 49.3% (302명)이었고, ≥10, <20 ng/mL에서 재발률은 31.4% (358명)로 유의한 차이를 보였다. 또한 Gleason Score는 ≥8점에서(ISUP grade group≥4) 54.4% (374명)의 재발률을, 7점에서 21.8% (720명), ≤6점에서 10.1% (115명)로 Gleason Score가 높을수록 높은 재발률을 보였다. 병리학적 결과로 surgical margin의 음성결과에서 재발률이 17.6%, 양성결과에서 재발률은 37.8%로 나타났다. PNI의 양성결과에서 재발률이 28.3%, SVI의 양성결과에서 재발률이 59%, ECE의 양성결과에서 재발률이 41.5%, LVI의 양성결과에서 재발률이 49.9%로 음성결과보다 재발률이 높은 결과로 통계적으로 유의한 차이를 보였다. BCR이 발생하기까지 평균 20.7개월, 중앙값은 15개월의 기간이 소요되었으며 최종 추적 관찰일은 전체 49.2개월, BCR이 발생한 환자는 54.9개월로 나타났다(Table 1).

전립샘암 생화학적 재발

근치적 전립샘절제술 후 BCR의 발생까지의 기간은 1개월 이내부터 최대 106개월까지로 환자에 따라 다양하게 나타났으며 BCR에 영향을 미치는 임상인자를 확인하기 위해 다변량 Cox 회귀분석을 시행하였다. 다변량 분석 결과 Gleason Score ≥8점, 그리고 PSA값이 ≥20.1 ng/mL가 중요한 예측인자로 나타났다. 또한 수술 전 PSA값은 ≥20.1 ng/mL에서(HR, 1.69; 95% CI, 1.44-1.99; p <0.001), Gleason Score ≥8점에서(HR, 3.43; 95% CI, 2.72-4.34; p <0.001), SM의 양성결과에서(HR, 1.29; 95% CI, 1.14-1.47; p <0.001), PNI의 양성결과에서(HR, 1.39; 95% CI, 1.19-1.62; p <0.001), SVI의 양성결과에서(HR, 1.47; 95% CI, 1.26-1.72; p <0.001), ECE의 양성결과에서(HR, 1.46; 95% CI, 1.28-1.68; p <0.001), LVI의 양성 결과에서(HR, 1.69; 95% CI, 1.48-1.92; p <0.001) BCR에 유의한 영향을 주는 것으로 확인되었다(Table 2). 또한 LML curve를 통해 비례위험 가정에 대한 검토를 실시하였고 그 결과는 Figure 2와 같다. PSA, clinical T stage 변수는 LML curve가 cross하는 경우로 다변량 Cox 회귀분석의 전제조건이 만족하지 않는 경우이다. 그래서 Time dependent Cox regression analysis를 통해 해당 변수에 대한 분석을 실시하였고 그 결과, 시간이 경과함에 따라 수술 전 PSA의 영향력이 차이가 있음을 확인하였다(Table 3).

BCR-FS와 비교분석

전체 BCR-FS를 분석한 결과 3년 BCR-FS는 79.6%이며, 5년 BCR-FS는 72.0%로 나타났다(Figure 3A). 병리학적 T 병기에 따른 3년과 5년 BCR-FS를 분석하였고 병리병기별 BCR-FS의 결과는 다음과 같다. T2에서 3년과 5년 BCR-FS 결과는 각각 88.2%, 81.9%이었고 T3a에서 각각 70.4%, 60.9%를, T3b에서 각각 44.8%, 35.0%, T4에서 50.5%, 37.5%이었으며 통계적으로 유의한 차이를 보였다(Figure 3B). 병리학적 T 병기별 각 변수들의 연관성의 통계적인 유의한 차이가 있었다(Table 4). 또한 병리학적 T 병기에 따른 BCR-FS 결과에서 수술 전 PSA, clinical T stage, Gleason Score, SM, PNI, SVI, ECE, LVI가 log-rank 결과에서 통계적으로 유의한 결과를 확인하였다(Figure 4).

고 찰

본 연구는 국소 전립샘암으로 근치적 전립샘절제술을 받은 한국인을 대상으로 하는 가장 큰 규모의 KPCR 전립샘암 데이터베이스를 활용하여 BCR 여부에 따른 임상병리학적 결과와 예후에 대한 다음과 같은 결과를 확인하였다.

첫째, PSA수치와 전립샘암의 5년 BCR에 대하여 기존 연구와 상반된 결과가 보고되었다. 서양인을 대상으로 한 Babaian et al. [18]의 연구는 PSA값이 ≤4 ng/mL에서 10%, 10.1-20 ng/mL에서 23.7%이었고 ≥20 ng/mL에서는 8.3%로 보고되었는데 이는 분석에 사용된 데이터가 수술 후 4.7%만이 PSA값이 20 ng/mL이었다는 한계점이 있었다. 한편 한국인을 대상으로 한 Kim [13]의 연구에서는 PSA값이 ≤10 ng/mL에서 15.2%, ≥10 ng/mL에서 43.3%를 보고하여 본 연구와 유사한 결과를 보였다.

둘째, 절제변연의 양성소견은 암 조절의 목표를 달성하기 위한 중요한 요소로 BCR의 중요한 예측인자로 보고되고 있는데 재발률은 연구마다 조금씩 차이가 있었다[19,20]. Dell’Oglio et al. [21]은 6,734명을 대상으로 분석한 결과에서 절제변연의 양성결과에서 재발률이 39.6%라고 보고하였고 Jhaveri et al. [22]은 33.8%의 재발률을 보고하여 37.8%의 재발률을 보인 본 연구보다 낮은 결과를 보였다. 절제변연 상태는 몇몇의 경우에 있어서 시행하는 수술기법에 따라 결과에 영향을 받을 수 있으며[23] 절제변연의 양성결과로 인해 예후가 악화될 수 있는데 기존 연구들을 통합해 보면 재발률이 33.8-39.6% 정도인 것으로 보인다. 향후 수술 전 적절한 수술 계획과 수술 시 세심한 수술적 절제를 통해 양성 절제변연을 피하는 노력이 필요할 것으로 보인다.

셋째, 근치적 전립샘절제술 후의 Gleason Score는 재발의 강력한 예측인자로 제시되며 점수가 높을수록 즉, 종양의 분화도가 나쁠수록 재발률이 증가하는 것으로 보고되고 있다[13-15,24,25]. 이렇듯 저등급 암보다 가장 나쁜 악성도를 가진 고등급 암 조직의 비율이 가장 높은 재발률을 보여 BCR에도 부정적인 영향을 끼치는 것으로 나타났다. 본 연구에서도 Gleason Score가 높아질수록 재발률이 증가하는 것으로 나타났으며 한국인을 대상으로 한 Cho et al. [26]의 연구에서도 Gleason ≤6점에서 16.1%, 7점에서 33.6%, ≥8점에서 48.7%의 재발률로 같은 양상을 보였다. Dell’Oglio et al. [21]의 연구에서는 Gleason Score ≤7점에서 71.6%, ≥8점에서 28.4%의 재발률을 보고하여 본 연구와 상반된 결과를 보였다. 근치적 전립샘절제술과 골반 림프절 절제술을 받은 대상자의 재발률에서 다른 점수보다 Gleason Score 8-10점에서 34.3%로 높은 재발률을 보고하여[18] Gleason Score 8-10점에서 54.4%의 높은 재발을 보인 본 연구결과보다 낮은 결과를 보였다. Gleason Score별 재발률은 연구마다 차이가 있었고 특히, 한국인에게 발생한 전립샘암이 서구에 비해 악성도가 높아 늦게 발견하게 되면 예후가 나쁜 경향이 있다[12].

넷째, 근치적 전립샘적출술 조직에서 정낭 침범이 발견되는 양성 정낭 침범의 생화학적 재발률의 분포는 58.4-67.1%로 보고되고 있고[8,26-28] Stephenson et al. [29]의 연구에서 정낭 침범이 10년 생존율의 예측이 가능하여 예후에도 많은 영향을 미치는 중요한 인자라고 하였다. 기존 연구에서 근치적 전립샘절제술 후 정낭 침범이 있는 환자에서 생화학적 재발률이 58.4%로 보고되었는데 본 연구 결과인 59%와 같은 결과를 보였다[8]. 앞서 기재한 생화학적 재발률의 결과보다 본 연구의 생화학적 재발률이 낮은 결과를 보였다.

다섯째, 전립샘암의 치료결과를 나타내는 지표로 사용하는 BCR-FS는 로봇수술을 받은 서양인을 대상으로 한 연구에서 5년 BCR-FS는 86.6-87.1% [15,16]이었으며 동양인을 대상으로 한 연구에서 5년 BCRFS는 72.4-80.1% [13,30,31]로 인종 간의 차이가 보고되었다. 본 연구의 BCR-FS는 3년에서 79.6%, 5년에서 72%로 10년 BCR-FS의 결과로 한국은 46.4%와 미국은 79%의 인종 간의 유의한 차이가 확인되었다[26]. 이러한 차이는 한국과 미국에서 국소 전립샘암으로 근치적 전립샘절제술을 받은 1,939명을 비교한 연구에서 임상적 특성을 보정한 후에도 한국인에게 발생하는 전립샘암이 서양인에 비하여 악성도가 높은 고등급의 암이 높은 것으로 확인되었다[12]. 이러한 결과는 예후에도 나쁜 영향을 주어 생존율의 차이가 발생하게 되며 결국 서양인을 대상으로 개발된 진료지침에서 벗어나 지리적, 인종적인 특성이 반영된 진료지침이 요구되는 셈이다.

본 연구의 한계점은 환자의 추적관찰기간이 평균 49개월에 불과하여 생존율에 대한 장기적인 예후 예측에 부족함이 있었다. 그럼에도 불구하고 본 연구에 쓰인 데이터베이스는 한국인을 대상으로 수집된 대용량 데이터베이스를 분석한 결과이다. 이 결과를 토대로 생존율 또는 재발률과 같은 예후 예측을 할 수 있는 시스템적인 지원이 따른다면 수술 예정 환자에 대해 정확한 위험도를 사정할 수 있고 이들에게 적절한 치료서비스를 제공할 수 있다. 이를 위해서는 필수적이고 표준적인 부분에 해당하는 요소를 일련의 과정을 통해 국소 전립샘암으로 수술받는 환자를 위한 재발예측의 시스템 프레임워크를 제공하는 것이 중요하겠다.

결 론

본 연구는 2003년 12월부터 2014년 12월 사이에 국소 전립샘암으로 근치적 전립샘절제술을 시행받은 환자를 대상으로 BCR에 관한 분석을 하였다. 국내 환자를 대상으로 한 연구 중 수술적 접근법의 데이터로는 가장 긴 추적기간을 나타낸다. 근치적 전립샘절제술 후 발생하는 BCR의 여부에 가장 영향을 미치는 인자로 Gleason Score가 ≥8점, 그리고 PSA값이 ≥20.1 ng/mL로 확인되었다. 이러한 결과는 대규모 데이터베이스를 통해 수술·병리학적인 여러 요소를 고려한 것으로 임상양상의 다양성이 반영된 결과이다. 이는 수술 후 추적 관찰 시 치료방향을 결정하는 데 도움을 줄 수 있으며 향후 생존결과의 개선에 기여할 수 있을 것이다.
